We are accepting new patient registrations.
If you wish to register, we will ask you to complete a registration form and a questionnaire. We will arrange a new patient registration medical check-up with a nurse (both for those who are well and those who have an immediate medical need).
Two forms of identification are required from everyone to register, one of which should be photo ID. To avoid delays, please keep a record of your NHS Number, this will allow us to identify and access your medical record from your previous GP Practice without delay.
We always require some form of confirmation of your home address at the time of registration e.g. utility bill, bank statement etc. and this along with a Photo ID will cover the two forms of identification. If you were born out with the UK we will also require confirmation of eligibility of NHS entitlement e.g. valid passport and visa; full-time student status letter, contract of employment.
